2nd – 5th Grade Kids Camp – Lassiter Volleyball

I, {name-2}, hereby give permission for my child, {name-1}, to participate in the volleyball kids camp at Lassiter High School on July 13-15. I understand that participation in sports involves physical activity and that there is a risk of injury. I acknowledge that my child is physically able to participate and will follow all rules and instructions provided by the coaches and staff.

I agree to release and hold harmless Lassiter High School, its staff, coaches, and volunteers from any liability for injuries or accidents that may occur during the tryouts. I also authorize emergency medical treatment for my child if necessary.
